Aetna Plans,Coresource,First Health,Coventry, Meritain Health, Cigna and Medicare. Please note Medicare only covers nutrition counseling for Type 1 and Type 2 Diabetes and Kidney Disease (pre-dialysis).
We work OUT OF NETWORK and PROCESS CLAIMS for:
UHC. If you have out of network nutrition counseling they reimburse between 60-80% of your visit.
We work OUT OF NETWORK and PROVIDE A SUPERBILL for you to submit for:
BCBS/Carefirst, Medicaid, Tricare, and anything else not listed above
Payment :Payment is due at time of service. Please notify Kindred Nutrition LLC of need to cancel appointment 48 hours prior to appointment by calling 301-580-0008. Email cancellations will not be accepted. Same day cancellations and no shows will be charged full visit.
** Please note**-
Kindred Nutrition is approved as an out of network provider for insurances. Clients will be provided with a Super bill after payment of their visit. It is the responsibility of the client to submit the super bill along with the appropriate medical claim form for reimbursement. It is also suggested to obtain prior authorization through your insurance company prior to your visit with Kindred Nutrition. You will ask the following questions: 1) Do you reimburse for out of network providers for nutrition counseling, specifically for CPT codes 97802 & 97803 2) What diagnosis' do you reimburse for 3) How many visits do you reimburse for in a year, 4) Am I responsible for a copay, co-insurance, or deductible and how much do you reimburse for each visit. If your insurance will not cover a visit with a dietitian you can utilize a flexible spending or health care savings account , cash, check, or credit card instead. Kindred Nutrition has limited sliding scale or hardship appointments available.
